Gastric cancer is a leading cause of cancer deaths, but analysis of its molecular and clinical characteristics has been complicated by histological and aetiological heterogeneity. Stomach cancers are 90% adenocarcinoma; lymphoma, carcinoid, and stromal tumours may occur. Adenocarcinoma can be subdivided into histological Lauren and the World Health Organization (WHO) classifications, but this information has not led to the development of histologic subtype-specific treatment options. One way to potentially improve treatment for gastric cancers is to better understand the pathogenesis of this disease, the contribution of Helicobacter pylori infection, and host immune response to lead to the development of integrated histological and molecular classification schemes for gastric cancer. The hope is that these studies may facilitate the development of clinical trials to explore therapies in defined sets of patients, ultimately improving survival from this deadly disease.

The meeting that provided the material for this book was the 58th Symposium of the Federation of European Microbiological Societies (FEMS) entitled MOLECULAR PATHOGENESIS OF GASTROINTESTINAL INFECTIONS which was held in Helsingor, Denmark from 2nd to 4th September, 1990. The aim of this meeting was to bring together scientists from a range of discipline- microbiology, cell biology, molecular biology and immunology - to consider how microbes, including parasites, colonize and infect the gastrointestinal tract. The programme was designed to focus particular attention on the range of strategies whereby enterovirulent bacteria and parasites colonize the gastrointestinal mucin layer, how they adhere to and penetrate the epithelial layer by entering the cells or passing between them, and how various protein toxins may facilitate these processes. Speakers were especially encouraged to highlight the recent expansion in our knowledge of the molecular mechanisms by which enterotoxigenic and enteropathogenic Escherichia coli, shigellae, salmonellae and Yersinia enterocolitica cause intestinal disease. There were also discussions of recently-discovered gastrointestinal pathogens such as Clostridium difficile and Helicobacter pylori as well as accounts of how virulent determinants can be used to develop new diagnostic methods based on DNA gene probes and the polymerase chain reaction (PCR). These presentations provided the basis for the chapters in this book.

As someone who has spent nearly half his life wondering about the relationship between Helicobacter and gastric cancer, I find this textbook on the subject exciting and timely. In fact, I am not aware of any other volume that has been able to distil so much new knowledge into such a comprehensive account of a poorly understood field. Taking my own view, as a scientist placed in the middle of the spectrum between basic science and clinical medicine, I can see that the editors, Jim Fox, Andy Giraud, and Timothy Wang, provide a broad mix of expertise, which ensures that the subject is treated with the right balance. From clinicopathologic observations in humans, to epidemiology, through animal models, to molecular and cell biology, this team has hit the mark for most readers. Fox is a well-known leader in animal models with broad expertise. He pioneered the field with observations on Helicobacter species in animals, from the time when only one spiral gastric bac- rium was known, “Campylobacter pyloridis. ” Fox partners with Wang, whose team recently announced a dramatic advance in the field of carcinogenesis—the obser- tion that bone marrow–derived stem cells participate in the changes that become cancer. To this nice mix has been added Andy Giraud from my own country, who brings to the table some remarkable genetic models of gastric cancer based on alterations in the gp130/stat3-signaling pathway.

Bacterial flora in Crohn's disease tissues

Crohn's disease is an idiopathic, chronic, segmental and transmural inflammatory bowel disease. Three models implying a role for bacteria have been proposed: infection by a single, hitherto unknown pathogen; an unbalanced composition of the bowel flora; and an abnormally increased passage of bacteria through the bowel wall. We tested these hypotheses by an experimental approach combining morphology and molecular bacteriology. The test protocol consisted of microdissection of Crohn's lesions from histological slides, DNA isolation, purification and sequencing of bacterial DNA, and identification of bacteria based on these sequences. Legionellaceae were specific for Crohn. Ulcers in Crohn's disease were also enriched in Enterobacteriaceae. The bowel wall in Crohn's disease further contained larger numbers of bacteria than in controls. Overall, it is not yet possible to exclude the existence of a unique pathogen, such as in Helicobacter pylori-induced gastritis. Full-blown Crohn's disease is however clearly associated with important disturbances of the normal composition and spatial distribution of the intestinal flora.
